Count and percent of members included in a Required Priority Population for which the MCO, MCO’s subcontractors or other entities completed a comprehensive assessment during the measurement period, by age group and Required Priority Population category or Other MCO-Delivered Care Management. 

Other Entities include and are not limited to Community Mental Health (CMH) Programs, Special Medical Services, Home- and Community-Based Services (HCBS) case managers, and Area Agencies.
