Percent of members who received outreach from the MCO to enroll in care management within 30 calendar days of being identified as high-risk or high-need through a completed comprehensive assessment, by the outcome of MCO outreach. The measure is reported for four submeasures: A: Enrolled, B: Not Enrolled - Member Refused, C: Not Enrolled - Member Did Not Refuse & Attempts Completed, D: Not Enrolled - Member Did Not Refuse & Attempts Not Completed.
