Percent of members with a comprehensive assessment completed during the measurement quarter, where the assessment results were shared in writing with members of the local-based care team within 14 calendar days of being established. The local-based care team includes but is not limited to the member's PCP, specialist, behavioral health provider, and local care management entity.
